A persistent infection of the kidneys.
Causes of Pyelonephritis, Chronic
Recurrent bacterial infection of the kidney that may occur in both sexes and at any age, but those most commonly affected are elderly, incontinent (unable to control their bladder), and may have a catheter into the bladder.
Symptoms of Pyelonephritis, Chronic
Often no symptoms, but detected on a routine urine test. Some patients have vague loin pain, feel tired and pass urine frequently.
Tests for Pyelonephritis, Chronic
X-rays, CT and ultrasound scans of the kidneys, blood tests, urine culture tests, and cystoscopy (passing a flexible tube into the bladder).
Treatment for Pyelonephritis, Chronic
A very long course of the appropriate antibiotic, urinary antiseptics and alkalizing agents (eg. Ural, Citravescent). Patients are encouraged to drink large quantities of fluids. If a kidney abnormality is found, this may be surgically corrected.
Complications of Pyelonephritis, Chronic of its treatment
Scarring of the kidney, high blood pressure, anemia and functional failure of the kidney.
Likely Outcome of Pyelonephritis, Chronic
One third of patients cured by a six-week course of antibiotics, another third cured after six months of antibiotics and antiseptics, 10% progress to severe kidney damage and failure, and the remainder continue to have a chronic infection without symptoms or kidney damage.
